Dr. Saranya Wyles discussed cellular senescence as a key driver of skin aging, with markers like p16^INK4A correlating with wrinkling and perceived age. Large-scale single-cell sequencing revealed distinct fibroblast subtypes and divergent senescence pathways in sun-exposed versus protected skin. These findings support precision approaches to anti-aging therapies. Wyles emphasized shifting from “anti-aging” to “pro-longevity” strategies that combine regenerative and preventive methods to maintain healthier skin over the lifespan.

The Role of Angiosomes in Vascular Occlusions and the Use of Ultrasound with Soft Tissue Filler
Dr. Rebecca Fitzgerald explained how the angiosome model helps predict unusual necrosis patterns after filler complications. Neighboring angiosomes are linked by “true” and “choke” anastomoses, with choke vessels prone to spasm, influencing the spread of vascular occlusion. Cadaveric studies reveal frequent midline-crossing anastomoses in the face, explaining how emboli can travel to distant sites like the retina. Clinically, vascular compromise is driven by both emboli and vascular spasm, highlighting the need for cautious, slow injections.

Winning the Scar Wars: Treating Scars and Keloids with Lasers & Laser-Assisted Drug Delivery
Dr. Jill Waibel showcased advances in scar and keloid management, noting that multimodal laser protocols are most effective. PDL, fractional non-ablative, and ablative lasers target vascular, atrophic, and hypertrophic scars respectively, while laser-assisted drug delivery enhances outcomes with agents like triamcinolone, 5-FU, and PLLA. OCT imaging helps tailor laser settings to scar depth and vasculature in real time. She stressed prevention through early laser intervention, which can significantly reduce long-term scar formation.

Strategies for Successful Treatments in all Skin Types
Dr. Andrew Alexis reviewed aesthetic treatment considerations across skin types, stressing that structural differences in melanin, fibroblasts, and healing responses affect outcomes. In skin of color, pigment alteration is often a greater concern than wrinkles, requiring careful device choice, lower treatment densities, and pre/post-treatment regimens to minimize PIH. He urged providers to avoid assumptions about ancestry or phototype and to tailor approaches to individual goals. Prevention strategies like photoprotection, hydroquinone, and topical steroids are key adjuncts to safe procedures.
Treatment Pearls- Lasers for hair removal can also be used for other conditions, including BCCs and vascular lesions such as venous lakes. 
- Treatment of acne scars can be achieved using a combination of subcision and saline injections. 
- PP405 is a new-in-class, promising hair loss medication on the horizon. 
- A multimodal approach to periorbital hyperpigmentation is ideal and can include topicals, chemical peels, and 1927 nm laser.
