Reshaping the Face with Injectables Dr. Kimberly Butterwick, MD, FAAD shared her expert approach to reshaping the face with injectables. She highlighted the importance of a comprehensive patient assessment and a nuanced understanding of the facial anatomy and how the bone, muscle, fat, and skin individually age over time. There are four facial shape archetypes (angular, heart, oval, and round), and each ages uniquely. An evaluation of facial beauty requires an analysis of volume, shape, contour, symmetry, projection, shadowing and proportions. She had a few simple takeaways for the audience. 1) If the face is relatively wide, add anterior projection, 2) If the face is narrow and long, widen the midface and/or jawline. 3) If the face is short, elongate the chin. 4) To enhance female features, widen the midface versus jawline, narrow the jawline with neuromodulator, narrow the chin with anterior projection, or enhance/balance lips. |

Controlled Subcision, Energy-Based Devices, and Injectables to Tackle Cellulite We now have a diverse armamentarium of devices and injectables to treat both the dimples and laxity which contribute to cellulite, a great concern to patients. Dr. Jeremy Green, MD, FAAD highlighted that effective treatment usually requires a combination approach. Our ability to treat dimples has significantly improved, with noninvasive devices such as rapid acoustic pulse or super selective devices such as targeted verifiable subcision. Laxity on the other hand can be improved via biostimulatory injectables and/or thermal tightening technologies. He underscored the volatility of the cellulite space, referencing the recent halt in the production of injectable collagenase clostridium histolyticum. Nevertheless, great results can be achieved via proper patient selection, expectation management, and a combination approach to contours and laxity. |

New Devices You Will be Using in the Next 1-3 Years that You Haven’t Heard Of Dr. Mathew Avram, MD, JD, FAAD presented an overview of three potential technologies well-positioned to transform the landscape of aesthetic medicine in the next couple of years. A new genre of laser, labeled the “4D Platform Technology” may enable skin of color to be treated more effectively and safely. Unlike the collimated beam of a prototypical laser that affects all tissues in its path, the device utilizes a novel highly focusable ring energy beam that precisely delivers higher fluences into the dermis at selectable depths and lower fluences to the epidermis, minimizing the risk of complications. Moreover, it incorporates a high speed, high resolution imaging system that intraoperatively guides the location and dosing of ideal energy and personalizes treatment by skin type. The tunability of the device enables the treatment of superficial dyschromias as well as deeper indications such as wrinkles, scars, and laxity. Dr. Avram also presented an update on Dr’s Lilit Garibyan and Rox Anderson’s ice slurry technology for selective fat removal and a new smart sensing needle that will enhance the safety of dermal filler injection through real-time LED feedback of potential vascular occlusion. |
